When I'm on the night shift, all the department heads can't sleep.
Chapter 211 Sign this chapter yourself.
Before the Monday morning shift handover was even finished, Lin Ye affixed his new employee badge to the card reader next to the emergency room doctor's workstation.
The screen flickered, and his name quickly appeared in the upper right corner of the login page.
The name is no longer listed as a residency rotation; it has been replaced by "Emergency Department Resident Physician."
Previously grayed-out appointment requests, examination requests, and medical record signatures can now be clicked.
Ma Hao peeked over from the next computer and stared at it for two seconds: "This crappy computer finally recognizes you today."
"At least a few days earlier than you."
"Rubbing salt into someone's wound on the very first minute of work?" Ma Hao tucked his residency badge into his white coat, then, feeling it was too deliberate, pulled it out again. "Fine, you can be smug for now. I'll settle accounts with this computer after I take those three slots off."
Zhou Min had just finished handing over two observation beds from the night shift and was passing behind the two with her medical record folder in her arms: "I don't know if the computer recognizes people, but it definitely will after eight o'clock. Lin Ye, take on two beds in the general ward first. If you're unsure about anything, just call me. Ma Hao, stick to your own training arrangements and don't try to get everything done by Lin Ye."
"I heard you." Ma Hao slid his chair back. "You're a doctor now, get back to work."
Just after eight o'clock, the first triage form was placed on Lin Ye's table.
Guo Ming, 27 years old, right wrist injury.
The person came in carrying a backpack in their left hand and a bag of ice bought from a convenience store on their right wrist.
The bag wasn't tied tightly, and the dissolved water dripped down the back of his hand; he only dared to hold it with two fingers.
Lin Ye asked him to sit on the examination bed: "How did you fall?"
"I was moving goods in the warehouse and tripped. When I fell forward, I braced myself with my right hand." Guo Ming moved the ice pack away, revealing his swollen wrist. "Everything else is fine. Doctor, please prescribe some medicine if you can. They're waiting to unload the truck over there."
What time did you fall?
"Around six o'clock. I could still move at first, but it got more and more painful, so the boss asked me to come over for an X-ray."
Lin Ye supported his forearm, first examining the shape of his wrist, then checking his finger movements, sensation, and blood circulation to his fingertips.
There is no obvious deformity in the wrist; the swelling is mainly on the thumb side.
Lin Ye roughly pressed along the radial side of the back of Guo Ming's wrist, but Guo Ming dodged as soon as he touched it, so he couldn't pinpoint the most painful spot; Guo Ming's fingers could bend and straighten, but when he lifted his wrist, he immediately stopped because of the pain.
"Don't push yourself." Lin Ye put his hand back on the cushion. "Take an X-ray of your wrist, both anteroposterior and lateral views. Don't lift anything with this hand until the X-ray is ready."
Guo Ming glanced at his phone: "If everything's alright after filming, I can go back, right?"
"Let's see the results first, and also see exactly where your hand hurts."
Lin Ye finished filling out the inspection application and hovered the mouse over the submit button.
His name was automatically displayed in the electronic signature field, followed by the words "Resident Physician".
Qin Hai happened to be passing by the consultation room. Seeing him stop, he casually tapped the screen: "Still looking for the doctor guidance section?"
"I didn't look for it, I'm just not used to it yet."
"Then stop dawdling." Qin Hai lightly tapped the medical record folder in his hand against the edge of his desk. "This time, sign your own."
Lin Ye pressed submit.
A line of small print pops up in the bottom right corner: Lin Ye, Emergency Department Resident Physician, Signature Successful.
From next door came Ma Hao's drawn-out voice: "Signed?"
"It's signed."
"Electronic signatures are great; they cover up your terrible handwriting."
Lin Ye handed the examination report to Guo Ming and replied through the partition, "Xiao Ma, you should save your three cells first."
Ma Hao remained silent for two seconds behind the partition: "On the very first day, he took advantage of his status to bully people. He's not humble at all as a doctor."
Guo Ming took the examination report to the radiology department.
When he returned, the ice pack had leaked so much that it was unusable, so he stuffed it into the trash can.
He couldn't sit still; he would stand up to check the call number screen one minute and look down to reply to messages in the work group the next.
Several voice messages popped up on his phone in quick succession. He didn't speak them aloud, only replying with: "Wait for me another half hour. If not, you guys uninstall it first."
"Can't we move the goods without you today?" Lin Ye asked.
"I can move it, but I'll be paid half a day less." Guo Ming put his phone face down on his lap. "This time I'll be paid per truckload, only after I finish the job. Actually, it doesn't hurt that much, so please don't make things too complicated for me."
An image report notification popped up in the lower right corner of the workstation.
Lin Ye opened the X-ray and found no obvious interruption of the cortical bone on the anteroposterior and lateral views of the wrist. The report concluded that no clear signs of fracture were found in the bones of the right wrist.
Upon seeing the last line, Guo Ming immediately stood up from his chair: "I told you it wasn't broken. Get me some painkillers, and I can still catch half a bus back."
He reached out his right hand to grab his backpack, but as soon as his thumb opened, his wrist pulled back slightly.
Finally, I switched to my left hand and lifted the bag strap.
A notification appeared in Lin Ye's field of vision.
[Process Breakpoint: Examination results do not perfectly correspond to the location of tenderness]
The line of text stopped in my field of vision.
Lin Ye reached out and pressed his backpack back onto the chair: "Don't go yet. Just because the X-ray didn't show a clear fracture doesn't mean the location of your pain has been explained."
"But the report says there's no fracture."
"It says no obvious fracture was found." Lin Ye pulled the cushion back in front of him. "Put your hand back up, and I'll check the area near the thumb again."
Guo Ming was still thinking about the cargo, but he eventually put his right hand back.
Lin Ye pressed along the back of Guo Ming's wrist, little by little, until he reached the indentation at the base of the thumb on the back of the wrist. Guo Ming then suddenly pulled his entire arm back.
"Is this the place?"
"Yes, it hurt when I waved my hand during the X-ray just now."
Lin Ye did not repeatedly press the same spot.
He steadied Guo Ming's wrist, grasped his thumb, and gently applied pressure along the long axis of his wrist.
Guo Ming took another breath: "It hurts inside, like it's hitting right here."
"When you fell, were your hands open or clenched into fists to brace yourself?"
"It was open. The person fell forward, and I just blocked it like this." Guo Ming made a bracing motion with his left hand, "My palm hit the ground first, and my arm went numb from the impact."
Lin Ye added the results of the re-examination to the record and then zoomed in on the regular film again.
"Are you still planning to go back and unload the cargo?"
"There's only half a truck left. I'll wear a wrist brace and use my left hand more forcefully; I should be able to manage."
What are you moving?
"A whole case of drinks."
Lin Ye pushed the backpack he had placed on the chair aside: "The wristband can't support this. This hand can't bear any weight today."
Guo Ming picked up his phone; someone in the work group was urging him to reply.
He stared at the message for a few seconds, then called back: "You guys uninstall it first, don't wait for me. The hospital still needs to do some more tests."
The person on the other end of the phone seemed to ask something, and he glanced at his swollen wrist: "It wasn't on the X-ray, but the doctor won't let me move it. You guys can disassemble it first, we'll talk about the payment later."
He hung up the phone, put it back in his pocket, and finally sat up straight: "Doctor, what other tests are needed?"
Lin Ye returned to the computer, opened the temporary fixation of the affected wrist on the treatment form, and then clicked on the wrist CT scan application.
"First, stabilize the wrist. Then, do a wrist CT scan to clearly see the scaphoid bone."
